I would never leave my name on the Internet on a survey nor my age, which is not relevant. Since I live in the US I don't know the pound/dollar conversion.

i'd prefer the age question to be something like age 10-20 or 20-30 instead of an exact age.

I usually give more than £20 when I donate - think about the audience of your survey. More than £20 might seem a lot to you but isn't so much for most adults.

One of the questions required an answer in pounds. For participants outside of the UK, other currencies would have helped.

Some questions were very confusing - especially the "don't you agree..."question. If I answer yes, then I don't agree the events are terrible.

Age question should be multiple choice. How often needs a time scale.
The amount given needs an open ended amount at the end.

Privacy issues re asking people for their real name and age - an age range would have been better. Wasn't sure of time scales on 'how often do you give'

For the amount, you haven't allowed people to give more than £20 - you could easily add >£20 as a category.

Some of the questions could have had more than one answer so then the feedback is less useful.
you could also have a q on how we donate?

Had a problem understanding double-negative. Didn't like giving too much personal info. Didn't understand time-scales. Didn't understand context of questions

I agree with the others that providing personal info like name and exact age is not a good idea. Also useful to find out how people give to charity.

Does giving to charity include 50p in a collection tin or is it limited to larger donations. You may give a few pence to someone in the street but does itcount

Might be good to say what you intend to do with the data - is is going to be kept secret or shared with others? Think about a privacy statement.

If the last question must be yes / no then maybe you should have those as the only answers - it would probably make it easier to sort the results.

Felt a bit strange giving my name - only submitted my first name. Wanted to give more info on how I give - ie through fundraising , subscription, charity box.

"How often do you give...". I give every 3-6 months but there's no answer that fits.

"Don't you agree.." is a confusing way to ask the question.

I also didn't know how often you meant I give to charity (e.g. once or twice a month etc). Leading questions too?!
